A Google Form gives you a great way to gather responses from people. In the properties section, specify whether you'd like to record the number as an Integer (whole number) or Decimal (0.0). Next, go to https://forms.google.com in a desktop browser and select the button the lower right to create a new form.

To automatically update from Acrobat, choose Help > Check for updates, and then follow the instructions in the Updater window to complete the update process. You can specify which options you present by clicking "Add Option."
